Grace Hospice, (Kansas City, MO), and Heart of America Hospice, (Topeka, KS), were acquired by Great Lakes Caring, a Jackson Michigan based home health and hospice company.  The transaction expands Great Lakes Caring’s Midwest operations into Missouri and Kansas.  Great Lakes Caring has 25 offices and services 9000 patients daily.  It is a portfolio company of Blue Wolf Capital.

Serenity Acres

Serenity Acres Treatment Center, a leading addiction treatment provider located outside of Annapolis, MD, was acquired by Meridian Behavioral Health, a portfolio company of Audax Private Equity.  The transaction expands Meridian’s footprint outside of Minnesota and facilitates its entrance into the Mid-Atlantic market.

Encompass Home Health Inc, a unit of EHHI Holdings Inc, acquired two home health care locations of Summit Home Health Care PC, a Casper-based provider of home health care services. The Braff Group provided exclusive merger and acquisition advisory services to Summit Home Health Care.

Infusion Resource

Specialty home infusion company Infusion Resource in Rhode Island was acquired by Amerita, Inc., a division of PharMerica. The transaction helps Amerita’s expansion into the eastern United States. The Braff Group provided exclusive merger and acquisition advisory services to Infusion Resource.

Spring Valley Hospice, LLC

Spring Valley Hospice, family owned local provider of hospice services, was acquired by Hospice Home Health, Inc. an Arkansas based provider of hospice services.  This transaction provides HHC with its first opportunity to expand its footprint outside its home state and into Mississippi.

Ed Support Services

Ed Support Services, LLC, one of the San Francisco Bay Area’s largest providers of educational and behavioral services for children with autism and related communication disorders, was acquired by Ben Klein, owner and CEO of Platinum Health Care, LLC.  Mr. Klein intends to partner with the remaining management owners to expand the company’s service offerings regionally and nationally.

Haven Home Healthcare, a dominant Dallas/Fort Worth provider of Home Health Care services, was acquired by LifeCare, a large regional owner of acute care hospitals designed to treat medically complex patients.  This synergistic acquisition allows LifeCare to extend its continuum of care into the community setting.

Always Best Care Senior Services

Always Best Care, one of the nation’s largest Private Duty Home Care Franchisors, was recapitalized by Gemini Investors and Plenary Partners.  The new partnership will be focused on accelerating its domestic franchisee growth and international expansion.

Oasis Hospice

Oasis Healthcare, a highly regarded hospice provider in northern Alabama, was acquired by Adobe Healthcare, a portfolio company of Frazier Healthcare Partners to expand and complement Adobe’s existing Alabama hospice business, Hospice of North Alabama.

Healy Pharmacy

Apothecary by Design, a portfolio company of BelHealth Investment Partners acquired Healy Pharmacy blending Healy’s strength in oncology and ABD’s experience in infectious and inflammatory diseases.

ReCept Pharmacy, a Texas-based specialty and retail pharmacy, acquired Mississippi-based F&M Specialty Pharmacy that focuses on servicing organ transplant patients. The acquisition provides ReCept Pharmacy a new geographic market as well as a new therapy line.

Lifematters, a greater DC metro provider of private pay home health care services, has acquired Care Options, a care management, home health care and support services provider. The transaction expands Lifematters’ capacity to offer “aging in place” service options to metro area residents and their families.

Care Unlimited, the largest provider of private duty nursing for medically fragile children and adults in western Pennsylvania was acquired by PSA Healthcare, a portfolio company of J.H. Whitney Capital Partners.  The transaction expands PSA’s footprint in PA.
