Beacon Health System

Great Lakes Caring, a portfolio company of Blue Wolf Captial, acquired the Home Health Care assets of Beacon Health Systems in Indiana and Michigan.  This transaction expands GLC’s existing patient base and service area.  The Braff Group served as Beacon’s exclusive mergers and acquisitions advisor.

Pyramid Home Health Services

Pyramid Home Health Services, a diversified home care services agency based in Missouri was acquired by Jordan Health Services, a large private equity-backed provider of Medicare, Medicaid, Commercial Insurance and Private Duty home-based healthcare services. The acquisition allowed Jordan to enter a new state and provide a full continuum of home health and home care services to nearly 4,000 patients statewide.

Excell Home Health Care

Excell Home Care & Hospice was acquired by The Cornerstone Healthcare subsidiary of The Ensign Group, a group of skilled nursing, rehabilitative care services, home health care, hospice care, medical transportation, & assisted living companies. This acquisition expands the geographic footprint of the home health & hospice service area into Oklahoma.

Comfort Hospice Care, a Nevada-based hospice provider, was acquired by Cornerstone Healthcare, a subsidiary of The Ensign Group. This acquisition extends Cornerstone Healthcare’s geographic footprint into the Las Vegas market and will allow the company to service patients needing hospice care in Las Vegas and surrounding counties.

Visiting Nurse Association of Ohio

The VNA of Ohio, one of the largest providers of Home Health, Hospice and Personal Care services in Ohio, was acquired by the VNA Health Group of New Jersey. The partnership will help both organizations deliver improved health outcomes for patients while expanding access, meeting the increased demand for post acute care, mental health, and population health services.

Hometown Home Health and Hospice

Hometown Home Health & Hospice, a large family-owned Home Health & Hospice agency, was acquired by Residential Home Health & Hospice, a subsidiary of Graham Holdings (NYSE: GHC). The acquisition supplements & extends the Michigan based service area for Residential.

Celtic Healthcare of Maryland, Inc.

The Baltimore and Maryland County locations, Maryland formerly owned by Celtic Healthcare were acquired by Texas-based Encompass Home Health & Hospice.  The acquisition includes the Certificates of Need and Medicare Provider Numbers.  This acquisition expands Encompass’ geographic footprint in the Mid-Atlantic region.

Medistar Healthcare Management Company

With the acquisition of MediStar Home Health, its 1,100 patients, and seven locations, Jordan Healthcare Services increases their existing certified home health coverage within the state of Louisiana and now enjoys the number one market share position accordingly.

Grace Hospice, LLC

Grace Hospice, (Kansas City, MO), and Heart of America Hospice, (Topeka, KS), were acquired by Great Lakes Caring, a Jackson Michigan based home health and hospice company.  The transaction expands Great Lakes Caring’s Midwest operations into Missouri and Kansas.  Great Lakes Caring has 25 offices and services 9000 patients daily.  It is a portfolio company of Blue Wolf Capital.

Summit Home Health Care

Encompass Home Health Inc, a unit of EHHI Holdings Inc, acquired two home health care locations of Summit Home Health Care PC, a Casper-based provider of home health care services. The Braff Group provided exclusive merger and acquisition advisory services to Summit Home Health Care.

Spring Valley Hospice, LLC

Spring Valley Hospice, family owned local provider of hospice services, was acquired by Hospice Home Health, Inc. an Arkansas based provider of hospice services.  This transaction provides HHC with its first opportunity to expand its footprint outside its home state and into Mississippi.

Haven Home Health, LLC

Haven Home Healthcare, a dominant Dallas/Fort Worth provider of Home Health Care services, was acquired by LifeCare, a large regional owner of acute care hospitals designed to treat medically complex patients.  This synergistic acquisition allows LifeCare to extend its continuum of care into the community setting.

Always Best Care Senior Services

Always Best Care, one of the nation’s largest Private Duty Home Care Franchisors, was recapitalized by Gemini Investors and Plenary Partners.  The new partnership will be focused on accelerating its domestic franchisee growth and international expansion.

Oasis Hospice

Oasis Healthcare, a highly regarded hospice provider in northern Alabama, was acquired by Adobe Healthcare, a portfolio company of Frazier Healthcare Partners to expand and complement Adobe’s existing Alabama hospice business, Hospice of North Alabama.

Care Options

Lifematters, a greater DC metro provider of private pay home health care services, has acquired Care Options, a care management, home health care and support services provider. The transaction expands Lifematters’ capacity to offer “aging in place” service options to metro area residents and their families.

Care Unlimited

Care Unlimited, the largest provider of private duty nursing for medically fragile children and adults in western Pennsylvania was acquired by PSA Healthcare, a portfolio company of J.H. Whitney Capital Partners.  The transaction expands PSA’s footprint in PA.

Sea Crest Home Health

Sea Crest, along with its sister companies, Sea Crest Hospice and Coastal Home Health were acquired by St. Joseph Health System based in Irvine, CA.  This acquisition greatly enhances St. Joe’s “continuum of care” and its ability to more completely service patients in its southern California service area.

Hospice of Kistsap County

Hospice of Kitsap County was acquired by the MultiCare Health System of Tacoma, Washington.  As a diversified system offering acute, post-acute, and other community based services, the deal builds out MultiCare’s hospice service offerings.

Ahava Hospice, a provider with four locations in South Carolina, was acquired by Homestead Hospice & Palliative Care, a growing company with 16 locations across 4 states.  The transaction expands Homesteads footprint in SC.

Heritage Home Healthcare & Hospice

Heritage Healthcare Services, the largest independent provider of Medicare and non-Medicare personal care services in New Mexico, was acquired by The Evangelical Lutheran Good Samaritan Society. The transaction advances the Society’s plan to grow their home and community based service offerings.

Encompass Home Health, a subsidiary of HealthSouth, acquired Harvey Home Health, a Medicare certified home health agency in Houston.  The deal will allow Encompass to further build on their strategic plan through ACO, BPCI, and other payor partnerships.

Angels At Home Health Care

Angles At Home Health Care was acquired by Great Lakes Caring, a portfolio company of private equity sponsor Wellspring Capital Partners.  With locations in Illinois and Massachusetts, the transaction expands Great Lakes’ regional footprint eastward.

Integrity Home Health

Integrity Home Health, a Las Vegas based provider of Medicare certified home health services with two locations serving Nevada, was acquired by Encompass Home Health, a subsidiary of HealthSouth.  The transaction expands Encompass’s already strong presence in the western US.

Focus Health

Focus Health, a home health services provider with 6 locations in southern California was acquired by Covenant Care, Inc., a large nursing home operator with locations in 7 states.  The transaction enables Covenant to offer a continuum of post-acute health care services.

Pri-Med Private Home Care

Pri-Med, a Chicago based infusion therapy and home health provider was acquired by KabaFusion, a multi-site infusion therapy and specialty pharmacy provider serving more than 30 states across the country.  The acquisition provides KabaFusion with a base of operations to serve the Great Lakes region.

Reach Therapy

Reach Therapy, a three-location pediatric therapy provider based in Texas was acquired by Professional Pediatrics Home Care, a portfolio company of private equity sponsor Bow River Capital Partners.  The deal is PPHC’s first and provides a toehold to support additional growth in the south central US.

Sanctuary Hospice

Hospice Advantage, Inc., one of the nation’s largest providers of hospice services and a portfolio company of private equity sponsor Sentinel Capital Partners acquired Sanctuary Hospice.  The deal provided Hospice Advantage with coverage in eastern and southeastern South Carolina, complementing their existing footprint within the state.

Community Hospice, Inc

Community Hospice, with six branches and an in-patient facility serving much of Mississippi, was acquired by Halcyon Hospice, a portfolio company of private equity sponsor Health Evolution Partners.  Headquartered in Atlanta, the transaction expands Halcyon’s regional coverage to SC, GA, and MS.

Residential Health Care Group, one of the largest home health and hospice providers serving Michigan was acquired by Graham Holdings Corporation (GHC), a diversified holding company with interests in health care, education, broadcasting, and print media.

Amedisys - Idaho Falls

Amedisys (NASDAQ:AMED), one of the nation’s largest providers of home health and hospice, engaged The Braff Group to divest approximately 50 underperforming locations.  The Idaho operations were acquired by EmpRes Management, a regional provider of senior housing and ancillary health care services.

VNA-TIP Homecare

VNA-TIP, one of the largest independent providers of home health, hospice, and private duty services across the state of Missouri, was acquired by Celtic Healthcare, a three-location home health and hospice company to expand its extend its regional footprint beyond PA and MD.

Human Touch Home Health Services acquired ASAP Services Corporation as a geographical expansion into nearby Washington D.C.

Horizons Hospice, LLC

Horizons Hospice, based in western PA divested its Virginia location to New Century Hospice, a regional consolidator of hospice providers.  Notably, New Century is headed up by the former president and CEO of Odyssey Hospice, a publicly traded, nationwide company subsequently acquired by Gentiva.

Addus HealthCare, Inc.

In late 2012, Addus Homecare (NASDAQ:ADUS), a publicly traded homecare provider, decided to divest its Medicare operations to focus primarily on Medicaid and other state-funded paraprofessional services and engaged The Braff Group.  The bulk of the business was sold to LHC Group (NASDAQ.LHCG) in 2013.

Lifecare Innovations

As part of its strategy to build out its home infusion therapy offerings, PharMerica Corporation (NYSE:PMC), a diversified national provider of institutional and specialty pharmacy services acquired Alternacare Infusion Pharmacy, a specialty home infusion provider serving the greater Kansas City metropolitan area.

Legacy Home Healthcare, LLC

Legacy Home Healthcare, a Prescott, AZ-based provider of Medicare-certified services, was acquired by The Good Samaritan Society.  Good Samaritan, a not-for-profit, multi-state provider of diversified post-acute services including senior housing, home care, rehab, and more acquired Legacy to complement its AZ holdings.

Living Care Hospice

Living Care Hospice was acquired by Compass Hospice, a multi-state provider based in South Carolina in order to establish a position in the attractive Georgia marketplace.

Horizons Hospice/ King of Prussia, PA

Many skilled nursing providers have begun to acquire hospice companies in an effort to broaden their post-acute service capacity. Publicly traded Brookdale Senior Living acquired a PA location of Horizon Hospice to fill a service need in this area.

AseraCare

AseraCare, a subsidiary of Golden Living, a skilled nursing home provider which had its original roots in Beverly Home Care is focused predominately in Hospice. The company engaged The Braff Group to divest its holdings of four underperforming branch operations providing non-core home health services.  Publicly traded LHC Group (NASDAQ:LHCG) acquired these locations as part of its continued consolidation strategy.

Freedom Hospice, LLC

Freedom Hospice was acquired by New Century Hospice which was founded by David Gasmire, one of the principle architects of Odyssey Hospice, an extremely successful provider that went public in the early 2000s.  After developing a substantial presence in TX, the acquisition of Freedom gave the firm their first foothold in nearby Oklahoma.

Advanced Homecare, LLC

Advanced Homecare, a Medicare Certified home health provider based in Florida, was acquired by Comprehensive Home Care, one of the largest, multi-site providers in the state.  The transaction expanded Comprehensive’s service area into the attractive Jacksonville market

Addus Home Health

In late 2012, Addus Homecare (NASDAQ:ADUS), a publicly traded homecare provider, decided to divest its Medicare operations to focus primarily on Medicaid and other state-funded paraprofessional services and engaged The Braff Group.  The bulk of these operations were acquired in a single transaction by LHC Group (NASDAQ:LHCG) to open up new service areas.

Amedisys (NASDAQ:AMED), one of the nation’s largest providers of home health and hospice, engaged The Braff Group to divest approximately 50 underperforming locations.  The Riverside and Lake Forest operations were acquired by VNA of Orange County, a multi-service provider of Medicare home health, hospice, and private duty home care to increase its penetration of the southern CA market.

Amedisys - TX

Amedisys (NASDAQ:AMED), a large publicly traded provider of home health care services, engaged The Braff Group to divest a substantial number of underperforming locations across the country. Stat Home Health, which provides home health services in Louisiana, and has a related and sizeable hospice operation covering the state, acquired five Texas home health operations as entry points into the neighboring state.

Amedisys (NASDAQ:AMED), a large publicly traded provider of home health care services, engaged The Braff Group to divest a substantial number of underperforming locations across the country. Celtic Healthcare, a business unit of The Washington Post, acquired the Williamsport, PA location to expand its footprint west from Pittsburgh into Central PA.

Amedisys (NASDAQ:AMED), one of the nation’s largest providers of home health and hospice, engaged The Braff Group to divest approximately 50 underperforming locations.  The Corpus Christi and Katy operations were acquired by Pathfinder Home Health, a large Texas-based home health provider to expand its presence in the eastern portion of the state.

Amedisys -TX

Amedisys (NASDAQ:AMED), a large publicly traded provider of home health care services, engaged The Braff Group to divest a substantial number of underperforming locations across the country. Pathfinder, a Texas-based multi-service home care provider, acquired the Dallas, Tenet, and Austin, TX locations to expand their capacity in TX. Pathfinder acquired locations in Texas in two separate, distinct transactions, which closed and funded at different times.

Amedisys (NASDAQ:AMED), a large publicly traded provider of home health care services, engaged The Braff Group to divest a substantial number of underperforming locations across the country. VNA of Orange County, a multi-service provider of home health, hospice, and private duty home care, acquired the Riverside and Lake Forest locations to increase its capacity in southern CA.

Great Lakes Caring, a PE sponsored provider of home health based in Michigan, acquired the Mentor and Toledo OH locations to increase their presence in nearby Ohio.
